• Immunization Information

    The State of Florida requires proof of immunization on all students entering the 7th grade. This proof must be on the official State of Florida Immunization Form (DH680) which is obtained from your doctor's office or the Health Department.
    The following immunizations must be documented on this form:
    1. Two doses of MMR (measles, mumps, and rubella)
    2. Tetanus, diphtheria and acellular pertussis (Tdap) or temporary certificate (680 part B) that lists the expiration date to return for Tdap if Td has been given.
    3. Hepatitis B vaccine series
    4. Varicella vaccine or year of disease
    It is recommended that you complete this requirement before the end of the 6th grade year to avoid any risk of your child's exclusion from school next year.
    What to do:
    1. Contact you physician to receive any necessary shots and obtain proper documentation.
    2. Submit this documentation (BLUE CARD) to the school office ASAP, and definitely BEFORE THE FIRST DAY OF SCHOOL, AUGUST 23, 2010.
    3. Your child will then be ready for the grade!!
    All the Grade students that are not up-to-date with the 7th grade vaccination requirements by the first day of school WILL NOT be allowed to enter school.
    There will be NO exceptions to this rule!!
    7th grade students will have to check in the the clinic on the first day of school.
